If you are a Health Care Professional who requires a copy of a patient’s ePCR for ongoing medical treatment, please be aware that records will not be released unless you provide Patient Consent for its release or another legal basis (under Articles 6 and 9 of the General Data Protection Regulation (GDPR) as to why you require the records without the provision of the patient consent.
Please complete the following form WAST Disclosure Request Form and return to amb.records@wales.nhs.uk. Your request will be assessed before the release of the record.
If the patient presented via your Emergency Department a copy of the ePCR would have been shared with them and should have been filed on the Patient Clinical records therefore, please check the patient records.
