Public Services Ombudsman (Wales) Act 2019 Notice

Notice pursuant to s.24(3) of the above Act.

The Public Services Ombudsman for Wales has investigated a complaint and found maladministration/service failure by the Welsh Ambulance Services University NHS Trust (The Trust) and has sent a report on the results of their investigation to The Trust.  The complaint related to:

  1. The handling of 2x 999 calls made to the Trust in respect of Mr B.

  2. The standard of record keeping demonstrated by the attending paramedic.

  3. Whether the arrival of the Trust staff at an earlier point would likely have affected Mr B’s outcome.

A copy of the report will be available on the Welsh Ambulance Services NHS Trust website www.ambulance.nhs.wales and for inspection by the public without charge during normal office hours at Welsh Ambulance Services University NHS Trust, Vantage Point House, Ty Coch Way, Cwmbran, NP44 7HF for a period of 3 weeks from 18th March 2025 and anyone who wishes may take a copy of this report or make extracts therefrom.

Photocopies of the report or parts thereof will be provided without charge during normal office hours.
